1) All the worlds could be different programs being run by a giant computer and Decade could be someone who was originally not a part of the computer but, when he went to try and 'unplug' everyone who was, he was pulled in and forgot he was not part of the program.

2) He could be a suit actor who had work in all the 10 previous Kamen Rider shows and then got hit on the head by Decade's henshin belt... thingy, and got his memories mixed up with that of the actual Decade

3) ... This could all be a dream meaning that chick's dream is a dream within a dream.
