For me it probably have to be The Gutter level. Due to fact that I missed The Pit/Grave of Saints level then somehow missed the first bonfire you can find in The Gutter level. So I basically had no way of getting out of the level safely and no torch to navigate my way around. It was stressful level as well due to every step could be your last and everything was freakin' dark.
So by the time I reached first bonfire I could find (probably second bonfire to most of people), I have already exhausted all spells I had, broken my main weapons with no way of repairing it, was out of flasks, and running low on other healing items. The level shouldn't have taken more than 30 minute first time through with torch but it took me more than an hour to get through I think. No way I would do the level again if I died. I was so determined to find bonfire.
So when I saw bonfire and relaxed. I realized I had been so tense for an hour and probably had hold my breath for that long ha.
Other moments would be having to fight that boss in Earthen Keep (sp?) without drain the poison from ground. Didn't know until after I beat the game you could fight boss without being poisoned and worse yet it healed the boss, lol. I also used human effinity thinking it would bring me back to human state to summon help, but nope for an hour, I can't get anyone to help for a while so I pitiful struggle until the effect lift. It didn't even bring me back to human state or do anything other than stopping the invasion for a while which I don't care for. I just needed help since I couldn't kill boss fast enough before poison killed me. Poor other phantom that I have dragged into. He must wonder why I didn't clear the area of poison, lol. Now I understand that messages were true when it said "torch" near the mill. I did try use torch but nothing happened for me back then, ah well.
Other one is battling the horse boss. That boss is probably one of my favorite bosses in the game. I was phantom when I first met the boss. Of course I didn't know to hide and nearly died when he ran through you. Controller vibrated each time he gets close which adds to more of tense moment. Love that battle. Literally a race to death.

Yeah, it's sneaky, but the npc summon for the poison queen was in the poison itself. So if you hadn't drained it, it was doubly harder.
